Comments:
An unannounced renewal inspection was conducted on November 16, 2023. The inspector was on site from approximately 1:00 pm until 2:50 pm. There were six children present with two caregivers supervising for a value of 13 points. Children were observed finishing lunch, playing and later napping. Today?s activity was an ice cream social and sensory project. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and four caregiver records were reviewed. The inspector interacted with caregivers and children in care. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The provider has submitted a written request to modify the age of children to include children three months to 12 years on the license. Standards regarding infant care were reviewed during this inspection. If you have any questions regarding this inspection, please contact the licensing inspector.

Based on observation, the provider did not ensure a landline telephone, excluding a cordless or cell phone, was available, operable, and accessible during the family day home?s hours of operation.
Evidence:
The family day home?s landline phone did not have a dial tone during the inspection on November 16, 2023 and was not operable.
Plan of Correction:
Per the provider: The phone worked earlier, I'm not sure why it's not working. I will have my husband check the wires to make sure it's working.
