An investigation was initiated on 04/15/2022 from 10:30 a.m. - 12:30 p.m. to investigate a complaint regarding behavioral guidance. Interviews were conducted, documentation, and records were reviewed. The complaint regarding behavioral guidance was determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on interviews it was determined that not all behavioral guidance was appropriate and constructive in nature. Evidence: On 03/10/2022 child A was not complying with directions and was attempting to move to another classroom. Staff B removed the child from the classroom bringing them to the school age classroom. Staff B can be seen on video roughly holding onto the arm of child A, and speaking closely into the child's face in an intimidating manner.
Plan of Correction:
Staff B was terminated because of this interaction with child A.
