Comments:
An unannounced monitoring inspection was conducted on 9/14/2023 at 2:05pm. There were 41 (infants to pre-K) directly supervised by 6 staff. The physical plant, 1 staff record, 5 children?s records, no new children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, waking up from nap, and interacting and doing activities with staff. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Based on review, the facility did not ensure that children's records were maintained with the required minimum information. Evidence: reviewed 5 children's records and found that Child 5 did not have an address for one of their emergency contacts.

Description:
Based on observation, the facility did not ensure that they had a sufficient amount of resilient surface around the playground equipment. Evidence: observed that the preschool play equipment had approximately 2-3 inches of resiliency covering the fall zones, and not the recommended minimum six inches.
