Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a tour of the program.
A monitoring inspection was initiated and concluded on 11/4/21. The director was contacted by email to initiate the inspection. There were 18 children present and 3 staff. The Inspector reviewed 3 children?s records and 3 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review it was determined the center did not ensure that each child had a physical examination by or under the direction of a physician within the required timeframe. Evidence: 1. The records for child #1, #2, and, #3 did not contain documentation of a physical exam. 2. Staff #4 confirmed that these records did not contain physical exam documentation.
Plan of Correction:
Director will request this documentation from parents and ensure each child's record contains documentation of a physical exam within the required time frame.

Description:
Based on observation it was determined the center did not ensure when food is brought from home the container is dated. Evidence: Lunch boxes were observed without dates.
Plan of Correction:
Director will remind all staff to make sure each lunch box is labeled with the child's name and dated daily.
