1-20-23 Arrived 2:00pm Departed 3:00pm An unannounced monitoring inspection was conducted today to review the previous violations which resulted in a provisional license. There were five children in care with three caregivers for a total of 16 points. The children were resting and eating a snack.
Information gathered during the inspection determined non-compliance with applicable standards or law and the violations were documented on the violation notice issued to the program.
Based on the review of staff and household member's files, the facility failed to ensure that all household members complete a sworn statement prior to the first day of residence. Evidence: 1. There is no sworn statement on file for household member #1. REPEAT VIOLATION
