11/3/23 Arrived 11:55am Departed 1:10pm An unannounced inspection was conducted today. There were nine children present, (ages 14 mos-4 yrs) for a total of 22 points, with two caregivers. The children were resting. Two child records, and two staff records, were reviewed. Thank you for your cooperation. Please contact me if I may be of service.
Based on a review of staff files, the facility failed to ensure that all background checks are conducted with in the required timeframes. Evidence: 1. There are no out-of-state central registry clearances on file for staff #1 & 2, both of whom indicated on their sworn statements that they had lived outside Virginia in the last five years.
Plan of Correction:
The provider reported that staff #1 & 2 will complete an out-of-state central registry clearance.
