The licensing inspector conducted a complaint investigation in response to a complaint that was received on 2/5/2024. The area of standards reviewed were administration. The inspection was initiated on 2/14/2024 and completed on 2/22/2024. Interviews with staff & collateral witnesses were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Based on staff & collateral witness interviews and documentation review the center did not ensure that children?s records shall be made available to parents on request.
Evidence: The director confirmed that on two separate occasions, the center did not make the record for Child #4 available to the parent when they requested it.

The file was made available to the parent prior to this inspection.
