An investigation was conducted in response to a complaint that was received by the department on May 16, 2023, and June 2, 2023 regarding allegations of the center being out of staff to children ratio. The inspector conducted an unannounced complaint inspection on May 18, 2023, and June 12, 2023. Staff interviews were conducted, classroom observations were made, and documentation reviewed. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. See the Violation Notice for findings.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on observation and staff interviews, the center exceeded the 1:4 staff-to-children ratio requirement for infants. Evidence: 1. On June 12, 2023, the 1:4 staff-to-children ratio was not maintained in the Infant classroom for approximately 30 minutes. 2. The director stated that there are usually 3-4 teachers in the infant classroom. Staff 3 had left to go on break.
