Comments:
A monitoring inspection was completed onsite on Thursday, October 28, 2021 from approximately 10:30AM until 12:30PM. The provider assisted the inspector during the inspection. There were three non-resident and three resident children present, ranging in ages from 11 months to two years with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-180-100-C
Description: Based on the review of two children?s records and interview, the provider did not ensure required information was in two children?s records.

Evidence:

1. The records for Child #1 (first date of attending 04/07/2021) and Child #2 (first date of attending 08/22/2021) did not contain the address of each parent or other responsible person or persons; address and telephone number of each parent's place of employment and his or her work hours; address one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care; the name of the parent's hospitalization plan and number or medical assistance plan; the parent's signed authorization for the child's emergency medical treatment; the parent's signed authorization to use a substitute provider and his or her name, address, and phone number; any written agreement made between the family day provider and the natural parent, guardian, or other responsible person for each child in care including hours of care per day, week, or month; cost of care per day, week, or month; frequency and amount of payment per day, week, or month; and any special services to be provided by either party to the agreement.
2. The provider stated the families have the model forms to collect this required information.

Plan of Correction: I will get the completed forms from the families and add the information to the records.

Standard #: 22VAC40-180-120-C
Description: Based on review of two children?s records and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.

Evidence:

1. The records for Child #1 (first date of attending 04/07/2021) and Child #2 (first date of attending 08/22/2021) did not contain a copy of the physical examination results obtained on each child before or within 30 days after enrollment; a copy of the Information to Parents statement and provide contact information regarding the inspector or contract agency; and the percentage of time that a substitute provider other than provider will care for their children. The provider stated the families have the model forms to complete to capture this information.

2.During documentation review, the provider stated emergency evacuation drills are not being completed monthly.

Plan of Correction: The parents have the forms and will add the completed forms to the records.
I will start doing monthly fire drills.
