Comments:
A monitoring inspection was completed on site on Monday, October 17, 2022. The licensing inspector was on site from approximately 9:30am until 11:15am. There were no children present during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of one child?s records, one caregiver's records, and three household members' records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of household members' records and interview, the provider did not obtain a search of the central registry for two household members that were 14 years of age.
Evidence: 1) The record for Household Member #1 and Household Member #2 did not contain the search of the central registry. 2) During interview, Caregiver #1 reported Household Member #1 and Household Member #2 turned 14 years old in December 2021 and the search of the central registry has not been obtained. The search of the central registry should be obtained within 30 days of a person in the home becoming 14 years old.
Plan of Correction:
The search of the central registry will be initiated for both household members.

Description:
Based on a review of one child's record and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence: 1) The record for Child #1, date of enrollment 04/07/21, did not contain a copy of the physical examination results before or within 30 days after enrollment. 2) During interview, Caregiver #1 reported a copy of the physical examination for Child #1 has not been received to date.
Plan of Correction:
The physical examination will be requested and maintained in the child's record.
