An unannounced renewal inspection was initiated on Tuesday, July 25, 2023 and concluded on Friday, July 28, 2023. On July 25, 2023, the inspector was on site from approximately 10:15am until 12:30pm. There were two non-resident children present during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of four children?s records, one caregiver's records, and three household members' records were reviewed. Additional documentation was reviewed electronically.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and returned it to me within five business days from today. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Based on a review of documentation and interview, the provider did not ensure one adult household member had the results of a fingerprint-based national criminal background check within the required timeframe.
Evidence: 1) The record of Household Member #3 did not have the results of a fingerprint-based national criminal background check. 2) During interview, Caregiver #1 reported Household Member #3 has not obtained the fingerprint-based national criminal background check to date.

Description:
Based on a review of documentation and interview, the provider does not meet all the criteria on the health and safety checklist at the time of the renewal inspection.
Evidence: 1) The provider did not keep a copy of the physical examination results obtained before or within 30 days after enrollment for one child. The record of Child #2, date of admission 01/16/23, did not contain a copy of the child?s physical examination results.
2) The provider did not have proof of adequate immunization received on the date of admission (or prior to admission) for one child. The record of Child #2, date of admission 01/16/23, did not contain proof of adequate immunizations. During interview, Caregiver #1 confirmed the immunization record and the physical examination for Child #2 have not been obtained to date.
3) The provider did not ensure electrical outlets were childproof in all areas accessible to children. During the inspection, the inspector observed four electrical outlets without protective covers. Two 2-year-old children were observed playing near the electrical outlets.
4) The provider did not ensure that any adult household member had tuberculosis (TB) test or screening. The record of Household Member #3 did not contain a tuberculosis (TB) test or screening.
