An announced monitoring and modification inspection was conducted on Monday, March 28, 2022 from approximately 9:35am to 11:35pm to review compliance with the requirements for Voluntary Registration in the providers new home. There were 5 children in care including the providers own child. A walk through of the home was conducted with the provider. Administrative records, emergency supplies and equipment and all areas of the home were inspected. Two children's and two caregiver records were reviewed. The provider stated that transportation is not provided at this time and medication is not administered.
Violations were found during the inspection and are identified on the violation notice.

Based on observation and record review, the provider does not meet all criteria on the health and safety checklist at the time of the monitoring inspection. Evidence: 1. The provider did not ensure electrical outlets are child-proof in all areas accessible to children. There were two uncovered outlets in the main room where children are in care. 2. The providers bathroom did not have soap and disposable or individually assigned towels. 3. The record of child #1 (start date: 6/15/21) did not have the following documentation: medical release form to permit emergency care; physical examination within 30 days after enrollment. Additionally, the provider did not provide an information to parents statement and notification of the fact that the family day home is covered by liability insurance. 4. The provider did not ensure to maintain a record for child #2 with all required documentation listed on the Voluntary Registration Health and Safety Checklist. Child #2 was present and receiving care in the home but the inspector was unable to determine the child's start date.
Plan of Correction:
Electrical Outlets - Corrected during the inspection. Bathroom Supplies - I corrected it. Child Record - Child will not be attending anymore.
