due to a state of emergency health pandemic declared by the Governor of Virginia.
An initial inspection was conducted on /4/21. The provider was contacted by video call to initiate the inspection. There were no children present. All documents required household members are on file with the department.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
