Technical Assistance:
1. Recommend the tearing/torn artificial turf be monitored to ensure it does not become a trip hazard to the children.
2. Reviewed recordkeeping requirements.
3. Inspector will send model forms.

Comments:
An unannounced monitoring inspection was conducted at 1:15pm. There were 7 children present = 18 points with the provider + assistant #1. Adult #1 was also present to assist during the inspection. Ratios and supervision were in compliance at the time of inspection. Children were napping during the inspection, while one child that was awake was playing quietly with the assistant caregiver. The daycare areas were well supplied with toys and equipment for the children. Areas were clean and supplies were stored in an organized manner. The daycare operates on the lower level of the home and has backyard play space. Information gathered during the inspection determined there was an area of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 8VAC20-800-500-C
Description: Based on observations made, it was determined that a piece of outdoor play equipment was installed over a hard surface. Evidence: A sliding board was placed on brick pavers covered with artificial turf. Areas of the turf were torn which exposed the brick pavers. There was no resilient surfacing present.

Plan of Correction: The slide will be removed from the area today and placed onto the grass until appropriate surfacing is installed.
