This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an on-site tour of the program.
A renewal inspection was initiated on February 15, 2022 and concluded on February 15, 2022. There were six children present, ranging in ages from 23 months old to four years old, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, two caregiver records and three household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of children's records, the provider did not ensure that each record contains all required information.
Evidence: The record for child #2 did not contain one of the two designated people to call in an emergency.

Description:
Based on review of record and interview with caregiver, the provider did not ensure that each caregiver had tuberculosis screening (TB) as required.
Evidence: The record for caregiver #2 did not have a tuberculosis screening available for review. An interview with caregiver #1 revealed that a TB screening or TB test results were not in the file for caregiver #2.

Description:
Based on observations, the family day home did not ensure all potentially poisonous substances, materials, and supplies are inaccessible to children.
Evidence: In room #1 there was a container of Lysol labeled ?Danger,? Harmful,? and "Keep Out of Reach of Children" and a bottle of bleach and water mixture. Bleach is labeled ?Harmful? and ?Danger?. Both containers of poisonous substances were located on top of the cubbies accessible to the children.
