Licensing Inspector conducted an unannounced (Initial/Monitor) Subsidy Inspection on November 13, 2023. There were 38 children ages 2 years to 10 years old present under the direct care of 6 staff. The Licensing Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on inspection, the Vendor failed to ensure electrical outlets have protective covers that are of a size that cannot be swallowed by children.
EVIDENCE: In the 2 year old classroom, Licensing Inspector observed several unprotected/uncovered electrical outlets.

EVIDENCE: A single swing was attached to the middle inner portion of the "dome climber". Equipment was altered and unsafe.
