Comments:
An unannounced monitoring inspection was conducted today from 9:08am-10:45am with the provider on site. There were 7 children (19 points) present with 2 caregivers. A sample of 4 children's files and 2 caregiver/household files were reviewed. The children were engaged in the following: playing with toys, washing hands, eating snack, sleeping and working on an Abeka work sheet. The site was organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, attendance records, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Based on review of drill records and interview, documentation was not maintained of emergency evacuation drills that included required information. Evidence: 1. The evacuation drills were not recorded for May, June, September, October, November and December-2023. The provider stated the drills were practiced, but did not write them down and only wrote down the shelter in place drills or lock down drills that month.

Description:
Based on observation and interview, the provider did not maintain documentation of the current rabies vaccination for a dog in the home. Evidence: 1. The provider stated she did not have documentation of the current rabies vaccination for the dog observed in the kitchen with children. The provide stated the dog was too old to have the vaccine.
Plan of Correction:
Due to inspection, the senior 2 Ib. toothless, miniature Chihuahua comfort dog had to pass through the kitchen while children were present to go outside. Otherwise, no contact with the children. Vet appointment will be scheduled. However, an allowable variance is being requested due to age, size, and breed.
