Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 11/08/2021 and concluded on 11/17/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 28 children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of three staff records and interviews, the center did not ensure that two staff records contained all the required information.

Evidence: 1) The tuberculosis (TB) screening results in the record for Staff #3, hired on 09/23/2021, were dated 11/10/2021. 2) During interview, a member of management acknowledged the TB results for Staff #3 were not obtained within 21 of beginning employment. Staff hired prior to 10/13/2021 were required to obtain TB results within 21 days of beginning employment and such documentation was to be retained in the staff record. 3) The record for Staff #2, hired on 09/30/21, did not contain TB screening results. 4) During interview, a member
of management reported Staff #2 does not have a TB screening to date.

Plan of Correction: Moving forward, staff will obtain the required TB results within the required time frames.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of three staff records and interview, the center did not ensure two staff members has a central registry finding within 30
days of employment.

Evidence: 1) The record for Staff #2, hired on 09/30/21, does not contain the results of a central registry finding. 2) During interview, a
member of management reported the request for the central registry finding for Staff #2 was sent on 10/13/21 and the results have not been
received to date. 3) The record for Staff #3, hired on 09/23/21, does not contain the results of a central registry finding. 2) During interview, a
member of management reported the request for the central registry finding for Staff #3 was sent on 10/07/21 and the results have not been
received to date.

Plan of Correction: In the future, the center will ensure to submit the request within 7 days of hire and contact OBI within 30 days of hire if the results have not been received. The center followed up with OBI for Staff #2 and Staff #3.
