Comments:
An unannounced monitoring inspection was conducted in the Family Day Home from 10:45 PM- 12:10PM. At the time of entrance, 9 children (age range 21 months -age 6) totaling 16 points were in care with 2 providers. The sample size consisted of 5children?s records and 2 provider records. Children were observed preparing for and participating in coloring activities and free paly in centers. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program
Based on record review and provider interviews, the provider did not ensue to maintain an up-to-date record at the family day home for each enrolled child with all the required information.
Evidence: The provider confirmed the following was missing from the children?s records as listed below; 1.The record for Child #2 and Child #4 did not include the insurance policy number of the child's medical insurance and the child?s first date of attendance 2. The record for Child #5 did not include the insurance policy number of the child's medical insurance, child?s first date of attendance, the name, address and telephone number of each custodial parent?s place of employment and the home address and phone number of one of the parents listed.

Description:
Based on record review and provider interviews, the provider did not ensue to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: Provider #1 and Provider #2 did not have documentation of updated TB screenings as the most recent screening for Provider #1 was dated 5-10-21 and Provider #2's screening was dated 5-20-21.
