Provider Registration and General Procedures Health and Safety Checklist
Comments:
An unannounced renewal inspection was conducted on 06/15/2023. There were 3 children, ages 15 months- 2 years, totaling 8 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of provider registration and general procedures and the health and safety checklist. A total of 4 children?s records, 1 caregiver record, 2 household member records and 1 pet record were reviewed. The children were observed in free choice play. The inspector arrived for the inspection at 9:30 am and departed at 11:15 am.
Information gathered during the inspection determined non-compliance with applicable requirements or law and violations were documented on the violation notice issued to the provider.
Based on record review, the provider failed to ensure that the record for each enrolled care in care shall contain all of the elements as required.
Evidence: 1. The record for Child 1 did not contain the following information: any known or suspected allergies and any chronic or current diseases or disabilities; and the child?s allergies to medication or drugs, if applicable, and directions for providing medicines to the child. 2. The record for Child 2 did not contain the following information: name, address and telephone number of each parent?s place of employment and his/her work hours (there was not an address or telephone number provided for the first parent listed). 3. The record for Child 3 did not contain the following information: results of the health examination.
