Comments:
A Voluntary Registered (VR) monitoring inspection for the modification of the VR Certificate for the relocation to a new address was initiated and concluded on 1/12/2023. There were no children present or enrolled at the time of the inspection. The inspector reviewed compliance in the areas of administration, physical plant, medication, special care and emergencies and nutrition. Areas to include staffing and supervision and programming were discussed since they could not be observed without children enrolled. A total 2 provider and household member records were reviewed. The inspection started at 2:15 p.m. and concluded at 3:40 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.036-B-2
Description: Based on a review of records and interview, the provider did not ensure that each household member, 18 years of age or older, submits the results of a Criminal Record Search.
Evidence:
The record for household member #2, who recently turned 18 years old, did not contain the results of a Criminal Record Search.
When asked if a criminal Record Search had been conducted, the provider stated that it was an oversight and had not yet been conducted.

Plan of Correction: The provider will contact the Office of Background Investigations (OBI) and register an appointment for household member #2 to have fingerprints taken for a Criminal History Records Search next week (or at the earliest available appointment).
Proof that the appointment was secured will be forwarded to the Licensing Inspector next week and the results will be forwarded to the licensing inspector upon receipt.

Standard #: 8VAC20-850-110-C
Description: Based on observation and interview, the provider was unable to demonstrate that every criteria on the health and safety checklist was met at the time of the inspection.

Evidence:
The following Health and Safety criteria were not met at the time of inspection:
1. "I have taken steps to safeguard my home from open and obvious household hazards"
The water from the hot water tap in the restroom that children will use measured 127.8 degrees Fahrenheit with a digital thermometer at the time of the inspection. Water above 120 degrees Fahrenheit poses a burn risk to children if their skin comes into contact with the water.
2. I make sure that any adult (18 years of age or older)... has a Tuberculosis test or screening...."
There was not documentation available to demonstrate that household member #2 obtained a Tuberculosis screening. The provider stated that she did not have documentation of a TB screening for household member #2.

Plan of Correction: The provider will have the hot water heater adjusted not to exceed 120 degrees Fahrenheit by the end of next week (before children are enrolled).

The provider will have household member #2 make contact get a Tuberculosis screening next week (and will schedule to go at the next available date that the health department offers them).
