Technical Assistance:
Today we discussed adjusting the height of the mattress in the crib to protect the infant from being able to climb or fall out of the crib as she becomes able to pull up and stand inside of the crib.

Now that a child is enrolled in the program, the provider must practice with the child(ren) Monthly evacuation drills as well as 2 shelter in place drills annually (and Lock down drills to meet subsidy requirements).
The drills must be documented in order to demonstrate that they have taken place during inspections. A form for documenting these drills is available on our website.

Comments:
A monitoring inspection was initiated and concluded on 6/23/2023. There was 1 child present with the Provider. The point total was 4. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record and 2 caregiver / household member records were reviewed. The inspection started at 1:30 pm and concluded at 3:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:

Description: *REPEAT VIOLATIONP
Based on observation and interview, the provider was unable to demonstrate that every criteria on the health and safety checklist was met at the time of the inspection. Evidence: The following Health and Safety criteria were not met at the time of inspection:
"I make sure that any adult (18 years of age or older)... has a Tuberculosis test or screening...."
There was not documentation available to demonstrate that household member #2 had obtained a Tuberculosis screening. The provider stated that she did not have documentation of a TB screening for household member #2 at the time of the inspection.

Plan of Correction: The provider will have household member obtain a Tuberculosis Screening within 2 weeks and keep a copy of the results on file for review.
