Comments:
An unannounced monitoring inspection was conducted on 6/18/2024 from 11:15am - 12:55pm. At the time of entrance, 1 child was in care with the provider. The sample size consisted of 1 child?s record and 2 provider / Household Member records. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The child in care was observed being held, cuddled and rocked and trying to get to sleep in his crib. Medication administration was not observed. The provider states that she does not administer any medication and will not unless or until she completes Medication Training (MAT). Children are not routinely transported in a vehicle during care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-850-110-C
Description: Based on observation and interview, the provider did not meet all of the criteria on the Voluntary Registered Health and Safety Checklist at the time of the inspection.

1. There were three lightweight plastic shelving units that were of varying heights between approximately 4 feet and 5 feet tall. The shelves were not anchored and could easily be rocked with gentle pressure. Additionally, a large screened television was located on a low shelf and was also easily rocked with a light touch. A child could pull the shelf or television over on to themselves, if they tried to pull up by holding on to it or if they bumped into it.

2. The diaper disposal can was not hands-free operated. The provider would need to touch the receptacle with her hand, possibly spreading germs, in order to dispose of a soiled diaper.

3. The record reviewed for child #1 who has been enrolled longer than 30 days, did not contain the results of a physical examination. The provider confirmed that a physical was not present int he child's documentation.
