Comments:
A subsidy health and safety (SHSI) inspection was completed on 6/18/2024 from 11:15am - 12:55pm. At the time of entrance, one 12 month old child was in care with the provider. The sample size consisted of 1 child record and 2 provider / household member records.

A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined violations with applicable standards or law. Violations are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-260-A
Description: Based on observation, the licensee did not always maintain the areas and equipment of the family day home in a clean safe operable condition, to include unstable heavy equipment, furniture, or other items that a child could pull down on himself.

Evidence:
There were three lightweight plastic shelving units located in an area accessible to the 12 month old child in care, that were of varying heights between approximately 4 feet and 5 feet tall. The shelves were not anchored and could easily be rocked with gentle pressure. Additionally, a large screened television was located on a low shelf and was also easily rocked with a light touch. A child could pull the shelf or television over on to themselves, if they tried to pull up by holding on to it or if they bumped into it.

Plan of Correction: The provider states that the only child enrolled for care is not yet mobile. However, she will either anchor the shelves and television so that they are no longer unstable - or she will relocate them to an area where children in care will not have access to them.

Standard #: 8VAC20-790-390-F
Description: Based on observation, the provider did not have a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Evidence:
The receptacle used for disposing of soiled diapers was observed to be hand-operated.
The provider confirmed that she did not have a leak-proof or plastic-lined diaper receptacle that is foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Plan of Correction: The provider stated that she would purchase a hands-free operated diaper disposal system by the end of the week.
