Comments:
An unannounced Virginia 22.1-289.031 Code Compliance monitoring inspection was conducted beginning at 10:15 a.m. and ending at 12:45 p.m. with the director. There were 31 children in care ranging in age from 3 to 5 years of age under the care and supervision of 6 staff. Center documents and six staff records were reviewed. Classrooms and children's activities were observed.
Information gathered during the inspection determined non-compliances with applicable code sections and violations were documented on the violation notice issued to the program.

Based on review of documentation, the center did not describe in a written statement to the parents/guardians of the center and available to the general public requirement information. Evidence: There was no written statement regarding food services and public liability insurance.

Evidence: Staff #2 indicated out of state residence in South Carolina on a sworn statement dated 8/25/2021 and there was no documentation of an out of state central registry check for South Carolina, an out of state sex offender registry check for South Carolina or a criminal history check for South Carolina within 30 days of employment.
