Comments:
The licensing inspector conducted an unannounced renewal inspection on 2/8/2022 from 11:15am to 1:15pm. There were 9 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, medication, special care, emergencies and nutrition. A total of 3 children?s records, 1 household member record and 3 caregiver records and were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.
Based on observation, the provider did not ensure to have a working portable battery-operated weather band radio. Evidence: The provider did not have a working battery operated weather band radio.
Plan of Correction:
1. I will purchase a new radio right away. 2. Will do a monthly check on the radio and keep extra batteries. 3. [Provider]

Description:
Based on record review, the provider did not ensure one of three children's records contained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
