Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews. An in person inspection was conducted 10/12/2021 to inspect the home and review children records. A monitoring inspection was initiated on 10/12/2021 and concluded on 10/12/2021. The provider was contacted by telephone and some documentation was viewed virtually. There were 4 children present, ranging in ages from 4 months to 3 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records were reviewed. The provider and household records were reviewed at the initial inspection. Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the program.
Based on a review of children records, the Provider has not maintain all the required information in each child's record. Evidence: Child #1, first day of attendance 8/27/2021, does not have documentation of parents work place, address and phone number. Child #2, first day of attendance 8/27/2021 does not have documentation of physician name, address and phone number. Child #1, #2, #3, all first day of attendance 8/27/2021 does not have 2 emergency contact addresses.
Plan of Correction:
The provider will get the information from the parents and write it on the child registration form.
