Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 24, 2022, from 11:35 a.m. through 12:40p.m. Three children and ten staff records were reviewed and update. Areas of non-compliance may be found on the violation notice.

Based on observation, the center failed to have resilient surfacing under the outside playground equipment.
Evidence: 1. The outside playground was inspected. 2. There was no resilient surfacing under the playground equipment. 3. Staff confirmed no resilient surfacing was on the playground.

Evidence: 1. Ten staff records were reviewed. 2. Staff #8 did not have documentation of obtaining fingerprints before beginning employment. 3. Staff #8's date of hire was documented on 10/10/2022. 4. The director confirmed Staff #8 had not obtained fingerprints before beginning employment.
