Comments:
A renewal inspection was conducted on 06/04/2024 from approximately 9:15 a.m. to 11:45 a.m. No children were in care during the inspection. The program director was present for the inspection. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child (5) records and five (5) staff record were reviewed. The information gathered during the inspection determined violations with applicable standards or law.

Based on child record review, the provider failed to assure the physical examination was conducted within the required 12 months prior to attendance for children two years of age through five. Evidence: The physical examination conducted 11/17/2022 for child, C3, age 3 years, enrolled 01/09/2024, was conducted more than 12 months prior to attendance.
