Comments:
An unannounced monitoring inspection was conducted on October 5, 2023 from 3:20pm-4:00pm. At the time of entrance, 20 children were in care with 2 staff members present. The sample size consisted of 4 children?s records were reviewed. A staff record review was conducted prior to inspection. Children were observed during snack time, sharing current events in the group and bathroom breaks.
Areas of non-compliance are identified on the violation notice.
Based on record review , the licensee did not ensure each child has a physical prior or with 30 days of start date.
Evidence: Child #1 start date 08/28/2023. Child #1's physical was not available during the inspection.
