Comments:
An unannounced monitoring inspection was conducted on 1-24-2023 from 9:35am to 11:17am. There were 111 (3 months to 5 years) directly supervised by 19 staff. The physical plant, 2 staff records, 5 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. The facility did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed on the School-age playground, 3 broken bird houses with sharp edges, accessible to children. 2) observed in 3 preschool classrooms, the linoleum floor had large cracks that caused a tripping hazard.
