Comments:
An unannounced monitoring inspection was conducted today in response to a self reported incident that was received by the Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of lack of supervision. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of lack of supervision, and the incident is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days. Evidence: interviewed 2 staff members and found that Staff 1 and Staff 2 left Child 1 unsupervised in the classroom, while taking the other children to the playground. Staff were not aware that the child was missing until approximately 20-25 minutes later.
