Comments:
An announced monitoring inspection was conducted on March 20, 2024 from approximately 10:00 am to 11:45 am. The inspector reviewed compliance in the areas of administration, personnel, household members, physical environment and equipment, emergencies, caregiver training, preventing the spread of disease and nutrition. The provider record, 2 household member records, and 2 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected. There were no children in attendance.
The first aid kit was inspected and found complete. The menu, emergency numbers, and daily schedule were posted.
The information gathered during the inspection determined violations with applicable standards or law.
Violations were issued.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today, 03/21/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Based on record review, the provider failed to ensure that all individuals who live in the household shall obtain additional screening for tuberculosis every two years thereafter the initial screening. Evidence: The record for household member #1 contained documentation of a tuberculosis screening dated 05/18/2021. An additional screening was due on or before 05/18/2023.

Description:
Based on record review, the provider failed to maintain an individual record for each child enrolled in care. Evidence: 1. The provider did not have a record for child #2. 2. The record for child #1 did not contain the address and telephone number of each parent's place of employment and his or her work hours.

Description:
Based on observation, the provider does not meet the criteria on the health and safety checklist. Evidence: 1. Medications and toxic household products were stored in areas accessible to children. 2. The bathroom that the children use does not have disposable towels or individually assigned towels. 3. The temperature in the refrigerator was unable to be determined to be 40 degrees or less due to there not being a thermometer.
