Comments:
A monitoring inspection was conducted on May 1, 2023. The inspector was on site from approximately 4:15 pm pm until 5:25 pm. The director was present during the inspection. There were two children in care, ranging in ages from five years to nine years, with two staff supervising. Children were in the playing in the multipurpose room with scooters and balls. Later children had snack in the cafeteria and colored pictures. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed remotely on May 12, 2023. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on review of two children?s records, the center did not obtain documentation of a physical examination before the child?s attendance or within 30 days after the first day of attendance. Evidence: 1.The record of child #2, enrolled 03/30/2023, did not contain documentation of a physical examination, exceeding 30 days from the date of enrollment. 2. The record was reviewed remotely on 05/12/2023. The director stated she did not have a copy of the physical.

Description:
Based on interview, the center did not maintain and keep at the center a separate record for each child enrolled that contains all required information. Evidence: 1. On May 1, 2023, the licensing inspector requested to review two children?s records while on site at the center. Staff #2 stated children?s records, immunizations, and physicals were not kept on site. 2. The center did not have records for child #1 or child #2, both in attendance on May 1, 2023.the two children present on May 1, 2023, child #1 and child #2. 3. The director confirmed children?s records were being stored at the administrative office.
