Comments:
A renewal inspection was conducted on May 13, 2024. The inspector was on site from approximately 4:00 pm pm until 5:30 pm. The director was not present during the inspection. There were three children in care, ranging in ages from six years to 12 years, with two staff supervising. Children were in the cafeteria working on an art project making boats out of paper plates. Later children went to the multipurpose room and played with games with balls and hula hoops. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. An additional staff record and board officer record was reviewed on 05/28/2024 and 06/25/2024. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:
Based on review of four staff records, the center did not ensure that one staff member submitted documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within 30 days prior to employment.
Evidence: The record of staff #3, employed 05/02/2023, contained documentation of a negative tuberculosis screening dated 03/1107/08/2023. The tuberculosis screening exceeded the first day of employment.
