Comments:
A complaint investigation was initiated on January 5, 2024 regarding supervision, ratio, and staff qualifications. Interviews were conducted and documentation was reviewed. There was not a preponderance of evidence to support the allegation regarding supervision, ratio, and staff qualifications therefore the complaint was deemed not valid. Children were observed washing their hands, eating snack, and playing outside. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and nutrition. Five staff records were reviewed for qualifications and background checks.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Description: Based on review of staff record, the center did not obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.

Based on observation, hazardous substances were not kept in a locked place.

Evidence:
1. In multipurpose room, refill floor cleaning pads and hand sanitizer were observed on a table accessible to children. Two disinfectant sprays were found on an upper shelf in unlocked closets. Paint was found in a lower shelf in an unlocked closet.
2. In the school hallway, bleach and other hazardous substances were unlocked closets accessible to children.
3. In classroom #7, disinfectant spray was stored on an upper shelf of an unlocked closet.

Description: Based on interview, disposable diapers are not disposed of in foot-operated or in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence:
The 2 year old classroom had a hand operated disposable system. Director confirmed this equipment is used in all classrooms that conduct diaper changes.
