Comments:
A monitoring inspection was initiated and concluded on November 18, 2021. There were three children(10pts) present, ranging in ages from 7 months to 18 months, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, three of three records reviewed did not have written acknowledgement of receipt of all required policies and procedures. Evidence: 1. Child #1, Child #2, and Child #3's record did not have documentation of the Provisions of the Emergency Plan and the Liability Insurance Declaration in the record.
Plan of Correction:
The provider will print copies of each document and have the parents to sign the forms and document the record.
