Comments:
An unannounced monitoring inspection was conducted on September 12, 2023 from approximately 12:30 pm to 1:45 pm. Provider and two assistants were present with 9 children. Children were observed during naptime. Two child and three caregiver records were reviewed. Violations were found and are noted on the violation notice.
Based on review, provider did not ensure caregivers have TB tests or screenings every 2 years.
Evidence: The following was found during review of caregiver records: 1. The record for caregiver #1 had a most recent TB test/screening dated 7/8/2021 which is more than 2 years old. 2. The record for caregiver #2 had a most recent TB test/screening dated 8/5/2021 which is more than 2 years old.
