Comments:
An unannounced renewal inspection was conducted on May 20, 2024, from 9:50am-12:15 pm. At the time of entrance, ( 29 ) children were in care with (5 ) staff members present. The inspector reviewed 5 children?s records and 5 staff records. Children were observed during snack time, engaged in group Martial Arts, manipulatives, play dough and outdoor play time. Medications were observed. Areas of non-compliance are identified on the violation notice.
The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on DATE. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Based on observation, areas of the center is not maintained in a safe and operable condition. Evidence: In room #2, the Inspector observed a 2 inch hole in the right farthest wall below the outlet and peeling paint below the right window near the far right wall.
Plan of Correction:
The paint will be chipped off, sanded and repainted and the hole will bepatched.

Based on a review of medication and medication documentation, 1 child's medication did not include the medication authorization form signed by the parent and physician. Evidence: Child # 2 has a emergency medication on site for a food allergy. There is no medication authorization form signed by the parent and physician to administer this medication.

The medication authorization form will be signed by the parent and physician.

Description:
Based on a review of medications and medication documentation, the center has not maintained a prescription label in accordance with the prescribers instructions pertaining to the dosage, frequency and manner of administration. Evidence: Child # 3 has a medication on site for a food allergy, the medication was in a box but did not have a pharmacy label with the prescribers instructions pertaining to the dosage, frequency and manner of administration.
Plan of Correction:
The parents will provide a prescription label for the medication.
