Comments:
An unannounced monitoring inspection was conducted on 9/20/22. The provider had 4 children in care. two children were napping and one child was eating lunch at the start of the inspection. The older children had rice, chicken and fruit for lunch with milk to drink. A total of 12 points. The provider had a friend visiting at the time of the inspection. Three of the children, from two different families, were dropped of for the first time on 9/20. Lots of toys and space for the 4 children in care. A 5th child attends after school. A violation notice was given to the provider along with this summary. Based on the review of the physical plant the bottles for the two infants in care are not labeled to indicate the owner. Evidence: Bottles are not labeled with dates and names.

Description:
Based on the review of children's records, the provider failed to have the required paperwork for 3 out of 4 children. Evidence: Three children started on the date of the inspection and the provider gave enrollment paperwork to the parents at the time of drop off. The provider only has the phone numbers of the mothers of the children. No other information.

Based on observation during the inspection, the smoke detector in the child care area needs a new battery. Evidence: During the inspection, the smoke detector was chirping indicating that it needs a new battery.
