Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced renewal inspection was conducted on June 4, 2024, from 9:15am-11:15am. At the time of entrance, 4(8pts) children were in care with one caregiver present. The inspector reviewed 4 children?s records and 4 /staff/household member's records. Children were observed during toy play, puzzles, engaged in group activities and outdoor play time. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Description: Based on observation and interview, the findings of the current inspection were not posted in a conspicuous place on the licensed premises.
Evidence:
1. The family day home did not have the findings of the most recent inspection posted. The findings posted was dated 05/31/2023. The last inspection was 03/19/2024.
2. Caregiver #1 confirmed that the findings for the most recent inspection was not posted.

Plan of Correction: The provider will print the findings of the last inspection and post them today.

Description: Repeat Violation

Based on observation and interview, the first aid kit did not contain required supplies.
Evidence:
1. The first aid kit did not contain a first aid instructional manual.
2. Caregiver #1 confirmed that the first aid kit did not contain a first aid instructional manual.

Plan of Correction: The provider will purchase a first aid instructional manual and place it in the first aid kit.

Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.

Plan of Correction: The provider has contacted the parents to obtain a copy of the child's health examination form from the physician.

Plan of Correction: The provider will complete the Information for Parents Form and Liability Insurance Declaration forms and have the parents to sign it and document the record.

Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements.
Evidence:
1. Child #1's record(Start date-10/30/2023) did not contain documentation of additional immunizations. The last immunization record was dated 02/03/2022.

Plan of Correction: The parents have been contacted to get a copy of the child's updated immunization records from the physician.
