7/10/23 Arrived 8:35am Departed 9:53am An unannounced monitoring inspection was conducted today. There were 16 (ages 3, 4, 5years) children present with two teachers. The children were arriving, reading stories and playing outdoors. Three child records were reviewed. Two staff records were reviewed. Thank you for your cooperation. Based on a tour of the playground, the facility failed to ensure that there is resilient surface material under climbing equipment and equipment that has moving parts. Evidence: 1. There is less than one inch (of the required six inches) of resilient surface material under the swings. Bare ground is exposed. 2. There is less than two inches of resilient surface material at the end of the slide.
Plan of Correction:
New mulch is being ordered this week. We will put it down on arrival.
