Comments:
An unannounced monitoring inspection was conducted today at 10:27am. There were 4 children (12 points) directly supervised by 1 staff. The home, 3 children?s records, evacuation drills, children's medications, emergency supplies, and policies were inspected. Children were observed playing freely and participating in teacher led activities. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Based on observation, bottles were not labeled with the child's full name and the date. Evidence: Two infants were in care. One bottle was observed without a label to include the child's name or the date.

Based on record review, 3 children's records reviewed did not contain all required information. Evidence: 1. Child 1's record did not contain written authorization for emergency medical care. 2. Child 2's record did not contain one parent's name address, phone number for either home or work locations. 3. Child 3's record did not contain the child's health insurance policy number or addresses for at least two persons to contact in an emergency of the parents cannot be reached.
