Comments:
An initial inspection was Initiated and concluded on March 9th. This is a new facility so there were not any children in care during the inspection. All sections of the standards could not be observed today because there are not any children in care. Therefore, once a license is issued and within 60 days of receipt an unannounced monitoring inspection will be conducted at which time all areas of standards will be determined.

During the initial inspection all areas of standards were discussed and reviewed with the owners/program director, the entire physical plant inside and outside was viewed, and each classroom was measured. This facility is a two-story facility. Therefore, a stipulation stating that only children 3 years and up will be permitted on the second floor.

Violations:
Standard #: 22.1-289.058
Description: Based on observation the center failed to ensure that there was a carbon monoxide detector in the building.

Plan of Correction: A carbon monoxide detector will be obtained and installed.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center failed to ensure that areas are in safe condition.
Evidence:
- There is peeling paint on the handrails leading to the outside playground.
- There are two four foot tall soft sided shelves in the infant room that are not secured to a wall.
-There is a hole in the plaster under the banister at the top of the stairs.
- There is a hole in the plaster above the outlet outside the toddler area that is large enough for a child's fingers in.

Plan of Correction: All repairs will be made as soon as possible.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center failed to ensure that playground equipment has the required fall/use zones.
Evidence:
There are two pieces of swing, climb and slide equipment on the playground. Neither of these pieces of equipment has the required six feet of resilient surface under around into the fall/use zones of the equipment. Fall/use zones measure less than two and four feet five inches where a minimum of six feet is required.

Plan of Correction: The swing, climb and slide equipment will be removed and replaced with play equipment that does not include, climbing, sliding, swinging or moving parts.
