Comments:
A complaint was received and initiated on December 8, 2023, alleging ratio violations. An on-site investigation was completed on December 19, 2023. During the on-site investigation classrooms were observed, ratio was counted, staff were interviewed, and staff and children?s records were reviewed. The information gathered supports the allegation and the complaint is valid. Violations cited include ratios, attendance and safety belts in infant seats. The violations were discussed with staff at the center at the conclusion of the onsite investigation and appear on the violation notice.
Based on record review and interview the center failed to demonstrate that for each group of children, a written record of daily attendance is maintained that documents the arrival and departure of each child in care as it occurs. Evidence: 1 - Staff present in the building did not have any form attendance available for review. a. Documentation of children present in the build was hand written by staff upon request of attendance. 2 ? Staff present stated that they did not know how to access digital documentation of attendance that may have been available on the tablets available in each classroom.

Based on observations and interviews the center failed to ensure that required ratios are followed whenever children are in care During the investigation staff present in the infant room reported that from 12:30 to 1pm there were five children in care in the infant room with one staff member where two staff are required. a. Staff report that during the nap time staff go on break for lunch.

Based on observations the center failed to ensure that when a child is placed in an infant seat the protective belt shall be fastened securely. Evidence: Child #1 was observed awake in an infant seat without the safety belt engaged.
