Comments:
An unannounced renewal inspection was conducted on 5/19/2022 from 9:36am0 to 10:44am. At the time of entrance, 6 children were in care with 3 staff members present. Children were observed doing the following: playing with balls outside, washing hands and eating snack. The site was organized and contained developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills and attendance records were reviewed. Based on record review, one of two children's records did not have complete information. Evidence: 1. Child #1 (start date 1/06/2022) was missing two emergency contact addresses.

Description:
Based on interview, documentation of orientation training were not kept by the center. Evidence: 1. Staff #2 (Date of hire 2/18/2022) did not have orientation documentation in the file. Staff #3 stated the training was completed 3/02/2022, but did not have the document filled out or signed.

I will have the staff member fill it out today.
