Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 21, 2022 from 11:30 a.m. through 12:30 p.m. Upon arrival, there were 3 children in the care of the provider with points totaling 9. No medication is being given at this time. Three children records were reviewed and the record for one assistant.
Areas observed were: lunch, hand washing/bathroom procedures, emergency supplies, outside play area, and transition. The children appeared to be well aware of the daily routine and schedule.

Based on record review, the provider did not have a second emergency contact for 1 child.
Evidence: 1. Three children records were reviewed. 2. Child #3 did not have documentation of a second emergency contact. 3. The provider confirmed a second emergency contact was not given for Child #3.
Based on record review, one child did not have a complete physical address listed for a second emergency contact.

Plan of Correction:
I will contact the parents of Child #1 and Child #3 to obtain the missing information.
