Comments:
An unannounced monitoring inspection was initiated and concluded on October 18, 2023, to determine compliance with the requirements for Voluntary Registration. The licensing inspector was on site from approximately 9:30am until 11:00am. There was one nonresident child present with one provider. The child in care was five months old. The licensing inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. The inspector reviewed four children?s records and one provider?s record.
Information gathered during the inspection determines non-compliance with applicable regulations or law. Violations were documented on the violation notice issued to the program.

Based on review of four children?s records, the provider did not maintain the required information for one child in care. Evidence: 1. The record for Child #1 (First Date of Attendance: 09/07/2023) did not contain the address of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care.

I will add the required information to the child's record.

Description:
Based on record review and observation, the provider did not ensure to meet the criteria on the health and safety checklist at the time of the monitoring visit. Evidence: 1. The records for Child # 2 (First Date of Attendance: 09/17/2022), Child #3 (First Date of Attendance: 09/17/2022), and Child #4 (First Date of Attendance: 11/01/2022), did not include documentation of a physical exam. The records for Child # 3 and Child # 4 did not include documentation of physical exams and immunizations. 2. The record for Child #3 (First Date of Attendance: 09/17/2022) contained immunization records dated 10/02/2023.
Plan of Correction:
Going forward, I will ensure the children's records are complete prior to a child attending the family day home.
