Comments:
An unannounced monitoring visit and facility tour was conducted on 7/2/24. At the time of inspectors' arrival there were 15 infant to school aged children in care with 5 staff members. Children were observed interacting with staff, nap, and engaged in educational activities. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Based on observation and interview it was determined the center did not ensure that pillows and filled blankets were not used by children under two years of age. Evidence: 1. 2 of 2 infants in cribs had fluffy blankets in the crib with them. 2. Staff confirmed that they were unaware of the updated safe sleep practices.
Plan of Correction:
Director removed blankets from cribs during this inspection and will train all staff on current safe sleep guidelines.

Description:
Based on record review and interview it was determined the center did not ensure the record of children's serious and minor injuries contained all the required information. Evidence: 1. The injury report for child #6 dated 3/22/24 did not contain the date, time, and method of parental notification. 2. Staff confirmed this information was not included on this injury report.
Plan of Correction:
Director will ensure all information is reported on injury reports.
