An unannounced Subsidy and Health Inspection was conducted with in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Two staff record was reviewed for documentation of the mandatory Virginia Preservice Training and current certification in First Aid and CPR.

Based on a review of caregiver records, one assistant did not complete the Preservice Virginia Department of Education-sponsored orientation course within 90 calendar days of employment. Evidence: Caregiver A began employment in May 2022 and did not have documentation of the Preservice Training.
Plan of Correction:
The provider stated the staff would begin working on the Preservice training as soon as possible.
